How This Works

- 3 Print and fill it out completely by hand.
- 4 Bring the completed form and your supporting documents in person to: Civil Registry Office, Office of the Chief Administrator, Abyei Town.
This form cannot be submitted online — South Sudanese civil administrative processes currently require an in-person visit for identity verification and original-document review.
What You'll Need
- Completed application form (download below)
- Parent's national ID or valid identification
- Hospital or midwife birth notification, if available
- Two passport-sized photographs of the child (for children over 1 year)
Birth registration is free of charge and should ideally be completed within 90 days of birth. Late registrations are still accepted but may require an additional sworn statement from a local chief or community leader confirming the details of the birth.
