National ID Replacement Request

Apply for a replacement of a lost or damaged identity document.

How This Works

  1. 1 Review the requirements below and gather what you'll need.
  2. 2 Download the blank form using the button at the bottom of this page.
  3. 3 Print and fill it out completely by hand.
  4. 4 Bring the completed form and your supporting documents in person to: Civil Registry Office, Office of the Chief Administrator, Abyei Town.

This form cannot be submitted online — South Sudanese civil administrative processes currently require an in-person visit for identity verification and original-document review.

What You'll Need

  • Completed application form (download below)
  • Police report if the ID was lost or stolen
  • The damaged ID itself, if applicable
  • One passport-sized photograph

Processing typically takes 2–3 weeks. A temporary letter of identity confirmation can be issued on request while the replacement is processed.
